In our local area some establishments stated they would not accept the coupons off the internet and were not participating in a national offer. Politely let them know you enjoy their food (or products/services) and you are disappointed that they will not honor the coupon or certificate. Offer to give them your phone number in case they change their minds. More than one restaurant in our area changed their minds with a polite phone call.

Many of the establishments will require proof of ID for the birthday person so be prepared with your ID. Some Birthday treats will be valid only on the day of your birth, some are valid for a specific length of time so be sure and check the details of your specific offer. Look to see if there is an expiration date on your coupon.

Having said all of this; getting free things on your birthday can be a whole lot of fun (and save a lot of money), and many companies will go overboard and treat you with some extra surprises on your special day.
